釉原蛋白与牙釉质生物矿化

摘 要: 细胞外基质蛋白控制着牙釉质晶体的发生、成长和机化。釉原蛋白(amelogenin)就是其中重要的牙釉质基质蛋白之一,大约占釉质基质的90%。釉原蛋白通过自组装、蛋白酶解处理和晶体成核与成长的调控过程,调控着牙釉质的生物矿化。主要就釉原蛋白的结构及其在牙釉质发育过程中的功能作一综述,同时也对目前体外合成牙釉质类材料的方法作简单的描述。关键词:釉原蛋白;牙釉质;生物矿化

Amelogenin and biomineralization of dental enamel

Abstract: Extracellular matrix proteins control the formation of the inorganic component of dental enamel tissues. Amelogenin, the major structural protein of the enamel organic matrix, constitutes more than 90% of the enamel’s protein content. The basic and critical events including amelogenin assembly, proteolytic processing and crystal nucleation and growth during enamel development, regulate and control the biomineralization of enamel. The purpose of this review is mainly to evaluate the present state of knowledge regarding the molecular structure and the functional role of amelogenin in the regulation of crystal growth and the structural organization of the resulting enamel tissue. This review also provides a brief description of novel and bio-inspired approaches that have been used to synthesize enamel-like materials.

牙齿釉质的发育是生物矿化过程的典型范例之一,牙釉质晶体的发生、成长和机化根据基因蓝图编码的指令进行,这些指令由一些分泌到釉质基质中的蛋白质来执行,这些蛋白质通过其构成的基质

介导牙齿矿化的形成。釉原蛋白(amelogenin)就是其中重要的牙釉质基质蛋白之一。
都被表达,分别为AMELX (位于Xp22.3– p22.1)和AMELY (位于Yp11),但来源于X染色体的釉

从功能上讲,原蛋白基因表达的蛋白质占优势;
在mRNA和蛋白质水平,AMELX和AMELY之间
存在较少差异,但是在基因组的内含子区域存在较多的不同[4]。因此,在医学上常利用以上差异来辨别胎儿或尸体的性别。
在所有物种分泌的釉原蛋白中,主导亚型的相
1 釉原蛋白结构及功能特征
牙齿釉质细胞外基质蛋白主要由釉原蛋白与其裂解产物组成,釉原蛋白由内釉上皮的成釉细胞分泌,大约占釉质基质的90%[1-2]。人类X、Y染色体上都含有釉原蛋白基因位点,而且都含有7个外显子,对于男性,染色体X与Y来源的釉原蛋白基因

对分子质量大约为20 kDa,其中人类的由175个氨基酸组成,鼠的由180个氨基酸组成。全长釉原蛋白主要由疏水残基和相对较短的亲水C末端构成[5],其氨基酸序列基于组成不同可分为3个区:含45个氨基酸的N末端,富含酪氨酸,因此又称TRAP (tyrosine-rich amelogenin peptide);中心区,主要由Xxx-Yyy-Pro重复序列组成,具有疏水性;具有11个氨基酸的C末端,带电荷并具有亲水性。在所有哺乳动物中,C末端和N末端的氨基酸序列是一致的,有力地证实了N末端和C末端具有高度的保守性

,高度保守的氨基酸序列具有重要的生理
学相关性,这表明这些片段在釉质发生中起着很重要的作用。另外,釉原蛋白还有一个小的异形体——LRAP (leucine-rich amelogenin peptide),LRAP是釉原蛋白mRNA选择性剪切的结果。鼠LRAP含有59个氨基酸,由鼠釉原蛋白的33个N末端氨基酸和26个C末端氨基酸构成。LRAP具有信号分子的作用,能够改变靶细胞的基因表达谱,促进骨细胞的分化
[8]
和促进牙周组织的再生[9]。
与其疏水性结构相对应,釉原蛋白在生理条件下微溶于水[10]。研究发现,在pH=6.8、25 ℃、0.15 mol/L 离子强度条件下,重组鼠釉原蛋白(rM179)的溶解性最低,但是在较低或较高的pH值条件下其溶解性明显增强[11];
天然的猪釉原蛋白在接近中性条件下溶解性也是最低的。虽然天然的釉原蛋白
与重组的釉原蛋白(缺少N末端的甲硫氨酸和S-16的磷酸化)不同,但随着离子强度的变化,其溶解性也在变化,随离子强度升高而降低。目前,在生理条件下釉质形成的分泌期,其基质的离子强度和pH值已经被界定,pH值接近中性,离子强度相对
较高(大约165 mmol/L)[12]
。釉原蛋白具有较低的
溶解性,这易导致其形成聚集物,因此,釉原蛋白在生理状态下,很可能以固态或凝胶的形式存在,未溶解的釉原蛋白与溶解的釉原蛋白平衡共存[13]。由此可见,未溶解的釉原蛋白与溶解的釉原蛋白在早期釉质矿化过程中可能起到不同的作用。釉原蛋白这种溶解与聚集的特性与其蛋白质结构明显相关。
Renugopalakrishnan等[14]和Aoba等[15]通过圆二色谱法(circular dichroism spectroscopy, CD)和傅氏转换红外线光谱法(fourier transform infrared spec-troscopy, FTIR)研究溶液中牛釉原蛋白的结构变化,结果发现其二级结构主要由β-sheet、
β-turn和不规则螺旋组成。Goto等[16]于1993年完成了猪全长釉
原蛋白、片段及重组猪釉原蛋白的CD研究,结果发现釉原蛋白3种结构模式与其不同的氨基酸序列相对应:N末端(TRAP)为β-sheet;中心区富含展
开的聚脯氨酸 II 或β-turn,如β-螺旋;
C末端由不规则螺旋组成。关于其三级结构,Matsushima等[17]
利用同步辐射小角X射线散射(SAxS)技术对酸性溶液中20 kDa的猪釉原蛋白(缺乏C末端)进行结构分析,证明在酸性溶液中单个釉原蛋白分子结构呈不对称的狭长杆状或椭圆形。这种结构后来也被Fukae[18]进一步证实。这些扩展型的结构将加强蛋白质与蛋白质、蛋白质与矿物质的相互作用,这与釉原蛋白的作用是一致的。
2 釉原蛋白的自组装
由于釉原蛋白 cDNA的克隆及其重组蛋白在Escherichia coli中的成功表达、纯化,以及后来一些新的提纯方法的出现,高纯度重组釉原蛋白的获得为进一步研究蛋白质功能奠定了基础,推动了对其生物化学研究的深入
[11,19-21]
。在牙釉质形成的早
期阶段,釉原蛋白的自组装在调控釉质晶体生长和排列过程中起着至关重要的作用。1994年,Moradian-Oldak等[22]应用动态光散射技术(dynamic light scattering, DLS)、色谱法、原子力显微镜和透视电子显微镜研究发现,重组鼠釉原蛋白(rM179)自组装构成直径15~20 nm大小的“纳米球”状结构,与釉质晶体的侧面对齐,这些结构后来在鼠磨牙釉质发育阶段早期的组织切片中被证实,因此认为釉原蛋白“纳米球”为牙釉质晶体的发生和成长方向
提供了一个有机的微观结构[1]
。进一步研究证实,
这种自组装过程由其疏水性所驱动,而且对溶液的pH值和温度敏感,变化溶液的理化条件,釉原蛋白可形成直径5~100 nm大小的“纳米球”
[23-24]。 Wiedemann-Bidlack等[25]的研究也发现,釉原蛋白的自组装对pH值非常敏感,似乎也受其疏水性和亲水性相互作用的影响;更为重要的发现是,在生理pH、温度和离子强度条件下,全长猪釉原蛋白(P173)的磷酸化基团对其自组装成高度有序的链状结构有着潜在但却很重要的影响。2011年,Le Norcy等
[26]
进一步证明,釉原蛋白磷酸化基团对其自组
装有着很重要的影响。
Aichmayer 等[27]研究发现,悬浮状态的重组全长猪rP172和鼠rM179形成的“纳米球”呈椭圆形而非以前研究描述的球型颗粒,而且在接近生理条件下,能够进一步地集聚。这种椭圆形的“纳米球”

是构成高度有序各向异性的长链状结构的关键,同时证实这种长链状结构在纳米水平调控釉质晶体构成中扮演着重要角色,最终生成高度有序的平行排列的釉质羟基磷灰石 (hydroxyapatite, HA)晶体。
目前已证实釉原蛋白“纳米球”是发育中牙釉质细胞外基质蛋白的基本结构单位,其大小取决于凝胶状基质的组成和异质性。比如,从牙釉质发育阶段细胞外基质中分离的具有异质性的釉原蛋白碎片中,可探测到直径20~200 nm大小的“纳米球”,而在重组的全长釉原蛋白基质中,纳米球相对均匀(低异质性),其直径仅在15~10 nm之间[28-29]。从猪发育期釉质基质中提取的釉原蛋白包含7%的全
长蛋白、
11%的23 kDa蛋白和40%的20 kDa蛋白,还有少量多肽。在天然釉原蛋白基质凝胶中高异质
性(宽范围尺寸“纳米球”状结构)的存在主要是由于釉原蛋白酶解产物进一步融合造成的,这种融
合是釉原蛋白之间相互作用的结果[23]
。在釉原蛋白氨基酸序列157 aa ~173 aa之间可能为相互作用的
区域,这个区域也被Paine和Snead[30]通过酵母双杂
交实验证实,并称作自组装“B-domain”
。Moradian- Oldak等[31]研究发现,釉原蛋白“B-domain”的改变可导致其“纳米球”
直径大小分布从21 nm变化到49 nm,同时也发现这种“纳米球”
直径大小分布范围的增长是由“B-domain”区域改变造成
15~20 nm大小的“纳米球”融合而成。此外,这种融合现象也反映了天然牙釉质基质中含有丰富的釉原蛋白酶解产物和它们重新联合构成的异型体。
Fang 等[32]通过低温电子显微镜研究发现,釉原蛋白是逐步分层进行自组装,同时也发现其亲水C末端多肽之间的相互作用是低聚物构成和后续的分层自组装所必需的,这种自组装能稳定矿物质成核前的集聚,指导它们排列成线性长链再组成平行阵列,未成核的矿物质长链再融合构成针状的矿物质颗粒,最终形成成束的晶体。
牙釉质基质中丰富的釉原蛋白酶解产物与釉质基质中的蛋白水解酶有关,这些蛋白水解酶不同于骨、软骨、牙本质和牙骨质形成中的蛋白酶,因为这些蛋白酶不参与组织的重建,而是在釉质完成矿化之前逐渐地处理和最终消化釉基质蛋白(主要是釉原蛋白)。目前发现的釉基质蛋白水解酶主要有2种:金属基质蛋白酶溶牙釉质素( matrix metallo-proteinase, MMP)和丝氨酸蛋白酶(enamel matrix serine proteinase 1, EMSP1) [33-34]。溶牙釉质素对釉原蛋白酶解位点已被确认,位于F/S、
P/L、 P/A、 A/L、P/M、S/Q 和 W/L[35-36]。研究发现,MMP-20可以把全长人釉原蛋白(H175)酶解为TRAP、23 kDa肽段H164和H147,以及不同的C末端片段[37-38]。EMSP1主要在釉质成熟早期负责完全消化釉原蛋白。
3 釉原蛋白与矿物质的相互作用
目前,有两种策略来研究釉原蛋白与HA矿物质的相互作用,即测定吸附参数和晶体生长的动力学[39-41]。酶解后缺少亲水C末端的釉原蛋白对HA的亲和力下降,从而降低了它对HA的抑制作用,这种抑制作用的降低很清晰地表明,釉原蛋白与HA晶体之间存在着相互作用,酶解作用能够改变其相互作用
[39-40]
。目前有关釉原蛋白调控晶体生长
过程的分子机制假说包括:阻止晶体成熟前融合[42]、调控晶体形态和接下来晶体的伸长[43]。对不同釉原蛋白片段溶液中成长的HA晶体聚集物颗粒大小分布研究发现,全长釉原蛋白与缺少C末端的釉原蛋白相比,能更有效促进HA晶体的聚集,主要是两个参数——蛋白质对HA的亲和力和釉原蛋白自组装的倾向力的改变造成的,即紧紧相连的釉原蛋白纳米球”同HA晶体相互作用吸附到HA晶体上,把HA晶体连接起来并充满晶体之间的间隙,保护并阻止它们在成熟之前融合。就釉质基质分泌的早期阶段只有很薄的晶体构成来看,似乎这种保护作用是必需的,但是值得注意的是,釉原蛋白这种抑制作用要比其他的一些酸性大分子物质(如Phosvitin)要小,表明在釉质晶体形成的早期阶段,釉原蛋白的作用并不仅仅是抑制晶体的成长,还提供了保护性外膜的作用。其他研究结果也发现,最初形成的高度有序的晶体之间的距离大约为20 nm,这与全长釉原蛋白“纳米球”的直径匹配,故也支持了这一观点
[44]
。
磷酸八钙 (octacalcium phosphate, OCP)的晶体单位包含有像HA一样的结构和水层结构[45]。条带
样的釉质晶体形成初期是以OCP形式存在的,然后由OCP转变成HA[46]。因此,为了验证前面提到的假说——釉原蛋白调控晶体形态,有学者首先就釉原蛋白与OCP晶体的相互作用进行了研究。为了证明釉原蛋白对OCP晶体生长的影响,使用了两个不同的实验模型,第一个是双向扩散法[47],晶体的形成是在双向扩散盒中进行,将不同浓度(0%~2%)的釉原蛋白溶在10%明胶凝胶中。结果发现,含有釉原蛋白的明胶实验组其OCP晶体明显变长,而且釉原蛋白对OCP晶体形态的影响具有剂量依

赖性,釉原蛋白浓度越高,OCP晶体变长越明显。因此,作者分析釉原蛋白“纳米球”可能是同OCP晶体的010面相互作用致使晶体平行于其c轴快速生长,进而导致其明显变长。更有趣的是,在这个实验中,使用的釉原蛋白是猪发育期釉质基质的提取物,主要是由釉原蛋白的蛋白水解物组成,只有大约7%是全长釉原蛋白,因此表明釉原蛋白的C末端对晶体的形态没有特殊的影响,而是其他一些机制,如釉原蛋白疏水性和自组装特性被涉及。另一个实验模型是阳离子选择性透析法
[48]
,晶体的形
成是在阳离子选择膜和透析膜之间进行,用的是纯的10%的釉原蛋白凝胶。结果发现,当Ca2+通过膜扩散到晶体形成空间后,有非常显著的长的OCP晶体形成,长宽比率约为66。通过系统地测量晶体的厚度(a轴)、宽度(b轴)和长度(c轴),表明相对于白蛋白和明胶,釉原蛋白对OCP晶体形态具有明显和特殊的影响。长度与宽度的比率、厚度与宽度的比率的增长表明,釉原蛋白与晶体010面较强的相互作用抑制了晶体b轴的生长(宽度),生长抑制大小为b轴>c轴>a轴。釉原蛋白“纳米球”与OCP晶体各个面相互作用的程度取决于OCP晶体各个面(100面、010面和001面)所带电荷的量和暴露的水层的量,如100面含有一层水分子,这
就不利于疏水性釉原蛋白的黏附[45]
。因此,釉原蛋白的疏水性和亲水性、OCP晶体各个面所带电荷量
都可能影响釉原蛋白“纳米球”与OCP晶体的相互作用。重组全长鼠釉原蛋白(rM179)的效果明显不同于重组的缺乏亲水C末端的鼠rM166 釉原蛋白,提示釉原蛋白对OCP晶体形态的影响取决于
釉原蛋白有无亲水的C末端肽[49]
。
Kwak 等[50]利用天然的20 kDa猪釉原蛋白的
蛋白水解产物P148(在第16位丝氨酸处有一个磷酸基团)研究对钙磷的影响,结果发现当缺乏P148的情况下,无定型磷酸钙(amorphous calcium phosphate, ACP)相对较快地转化为碟状的HA晶体;与此形成鲜明对比,在存在P148的情况下,ACP相对较慢地转化为碟状HA晶体,并且发现可抑制磷酸钙沉淀和稳定ACP这种结构形式超过1 d。额外使用去磷酸化的P147和部分去磷酸化的P148的实验
结果都证明,
P148对矿物质的形成具有较强的影响。在后来的一些研究中也进一步证明了釉原蛋白中磷
酸基团的作用,天然的磷酸化的猪釉原蛋白(P172)同样能稳定ACP保持相对较长的一段时间,与去
磷酸化的rP172形成鲜明的对比
[25,51]
。在对HA晶体形态的影响方面,很早就有学者通过体外实验尝试评估釉原蛋白对HA晶体的作用。1986年,Weiner[52]研究提出,在生物矿化系统中,疏水性分子(如釉原蛋白)提供一个骨架或者叫可填充的空间结构,而酸性的亲水性分子(如Enamelin和Ameloblastin)主要参与调控晶体的成核和生长。但是后来有学者研究发现

,
25 kDa猪釉原蛋白能够吸附到HA表面,而且当增加到对HA过饱和的溶液中后,能部分地抑制其种子晶体
的生长。更为重要的是,23 kDa 猪釉原蛋白的蛋白水解产物20 kDa 、13 kDa 釉原蛋白大大降低了其
对HA的黏附能力和抑制晶体生长的能力。Wen 等[54]选择生物活性玻璃作为底物来研究釉原蛋白对HA晶体的影响,结果发现釉原蛋白能够影响置于过饱和钙溶液中的生物活性玻璃上形成的HA晶体的生长习性;重组全长釉原蛋白能够促进成束的长的
HA晶体的形成,其方向都平行于它们的c轴,这些成束的晶体中的单个晶体都呈狭长的弯曲的形态,而添加白蛋白则导致整个晶体尺寸的减少,表明其对晶体成长具有抑制作用。同时,该系统造成成束的碟状59 nm厚、300~600 nm宽的HA晶体定向成长,表明釉原蛋白是通过抑制HA晶体侧面生长和促使c轴方向优先生长来影响HA晶体的形态。Wallwork等[55]通过原子力显微镜可直接观察到沿着釉质晶体侧面附着的鼠釉原蛋白(rM179)“纳米球”。2012年, Beniash等[56]通过体外实验进一步发现,重组的全长鼠釉原蛋白和不含C末端的釉原蛋白都能抑制HA晶体生长(垂直于c轴的方向);同时,微晶构成的平行阵列仅在全长釉原蛋白存在下形成。这些结果表明,牙釉质蛋白的疏水部分在抑制晶体生长中起重要作用,而C末端结构域在晶体排列的取向中必不可少。从以上这些结果可推测,釉原蛋白参与了釉质发育过程中晶体生长的整个过程,但是,釉原蛋白与HA晶体相互作用的机制许
多方面仍不清楚,
尚值得进一步研究。Le Norcy等[57]最近也利用缺乏C末端的磷酸化和去磷酸化的富含
亮氨酸的釉原蛋白多肽LRAP,研究了疏水的N末端在ACP形成过程中的作用,结果发现在对照组中,ACP初步形成后,转化为随机取向的HA的板状晶体。相反,LRAP(+ P)-CT能稳定形成的ACP的时间为1 d,而LRAP(-P)-CT加快转变为HA,但对晶体的形状或方向几乎没有影响,研究结果也进一步显示,釉原蛋白的N端结构域在釉质形成的早期阶段发挥直接作用。

4 釉原蛋白在釉质仿生矿化中的应用
对生物分子介导的矿化过程的解读有利于设计和合成具有分层组织结构的无机材料。因此,由此获得启发合成复杂仿生材料的方法引起了材料学、工程学和牙科学等多个学科学者的广泛兴趣。通过这些方法不但能够提高对生物矿化过程的认识,而且为合成具有理想理化特性的功能性无机复合材料提供了一种路径。生物材料,如牙釉质,具有复杂的结构和突出的物理性能,通过传统的材料合成方法(大多是在高温或低pH条件下)很难获得,开发一种不需要极端理化条件(如pH和温度过高或过低)的仿生合成方法变得越来越重要
[58]
,如自组
装有机超结构作为模板引导和调控无机材料(如磷酸钙)的生长[59];
通过与高分子骨架(大多是蛋白质)相互作用来合成构建生物硬组织 [60-61] ;
利用晶体生长水热法合成类牙釉质样结构等[62-63]。近几年来,对釉原蛋白的结构、功能、基因表达模式、釉质形成过程中矿化期本质的认识以及釉质有机基质之间的相互作用认识的发展,使得人们能够在实验室中利用一些技术在无细胞条件下合成像釉质样的材料,而对蛋白质自组装和同时发生的钙磷结晶调控的认识是设计和开发釉质类材料的基础[64]。最近几年在该领域也取得了一些有意义的进展,比如前面提到的重组全长釉原蛋白在过饱和钙溶液能够促进成束的纵长的HA晶体的形成,其方向都平行于它
们的c轴等[48]
,但目前仍不能完全复制釉质的详细结构和功能特性。
5 总结与展望
釉原蛋白是釉质形成中重要的釉基质蛋白,其化学结构、自组装和与矿物质的作用关系在调控釉质矿化过程中起着作用。在哺乳动物中,高度保守的氨基酸序列表明,这些片段在釉质发生中起着很重要的作用,其疏水性结构导致釉原蛋白在生理条件下具有较低溶解性;全长釉原蛋白的自组装结构——“纳米球”在调控晶体形状与构成中起着独特的作用,但受其釉原蛋白二、三级结构的影响;与OCP、ACP、HA的相互作用调控着晶体的成核与生长。基于目前已经确定的这些研究成果推测,全长的釉原蛋白自组装、与矿物质的相互作用以及与其他釉基质蛋白协同调控釉质晶体的形成、成长和排列等事件最终形成了几乎完全没有基质蛋白存在痕迹的、高度矿化的、高度有序的、具有特殊结
构的成熟釉质。很显然,需要更多的研究来阐明和证实这一过程,如证明全长釉原蛋白在矿物质形成期是否能够构成定向的高度有序的结构;为什么蛋白水解过程是必需的;钙离子在釉质基质自组装中的角色;对矿化驱动力的调控;其他釉基质蛋白的潜在角色等等。阐明和证实这一过程,对于牙釉质的再生和研制新型的牙科修复材料具有重要的价值和意义。
